Two possibilities: one is if you've recently been hospitalized, they're probably checking to see if you've kept your appointments, gotten your medication, know how to use your O2, etc. If it's just a "wellness visit" they may do much the same thing. Either way, take advantage of it!

It's usually for an assessment after a problem you had that may or may not have put you in the hospital.

I have a Heath care worker she calls me to see if she can help me with anything like making sure I get my med and alot she cheers me up so if it's like what I have there to help u out
